What the research says about ostarine
Ostarine was developed by GTx as a treatment for muscle wasting, and unlike most compounds in this category it reached genuine clinical trials. The best known is the phase II study reported by Dalton and colleagues in the Journal of Cachexia, Sarcopenia and Muscle in 2011, which gave 159 healthy elderly men and postmenopausal women enobosarm or placebo for twelve weeks. Total lean body mass increased in a dose related way and stair climbing power improved. Doses used in that published research ranged from 0.1mg to 3mg daily.
Dobs and colleagues published a further phase II trial in Lancet Oncology in 2013 in patients with cancer related muscle loss, again reporting increases in lean body mass against placebo. The later phase III POWER trials in non small cell lung cancer met their lean mass endpoint but did not meet the co-primary physical function endpoint, which is the reason enobosarm was never licensed. That context is usually left out of retail copy: the compound did what it was designed to do to tissue mass and still failed to show the functional benefit regulators wanted.
The pharmacology is straightforward. Ostarine is a non steroidal partial agonist at the androgen receptor with a preference for muscle and bone over prostate tissue, which is what the word selective in the name refers to. Reported findings across the trials include reductions in HDL cholesterol and in sex hormone binding globulin, and suppression of endogenous testosterone at the higher doses studied. Case reports of drug induced liver injury associated with ostarine containing products have appeared in the hepatology literature, though in several of those cases the product was never analysed.
